The intake process is much faster if the forms are filled out prior to your (or our) arrival for the massage session. Please print and fill out the Health Information Form, the Screening Questions, and the Body Map for Clients form. If you are not able to do so, let us know that you will be completing them on site so we are able to schedule the time for you to complete them and still have time for your full session.
